You will need two additional forms when completing the Self-Elevating Platform (Jack-up) DP scheme. These are:
- The additional taskbook which replaces the taskbook in Phase B of the logbook and,
- The DP Operations form where the breakdown of the operations can be recorded to provide evidence.

How do I complete the logbook for the Self-Elevating Platform (Jack up) Scheme?
If you decide to choose the Self-Elevating Platform (Jack up) Scheme, you will be given a grey offshore logbook by the training centre when you have successfully completed and passed the DP Induction Course. You will also need the additional taskbook, and the DP Operations form.
The Phases that you will use will be as follows:
Phase A: The DP Induction Course will be stamped and signed in the logbook by the training centre. This must be completed before starting on Phase B.
Phase B: The 60 days on board the vessel can be added to the Phase B in the logbook. The 15 DP operations required for Phase B must be recorded on the additional DP Operations form. The additional form can be found here.
The separate taskbook will need to be completed. This is due to some of the tasks being adapted to the different type of scheme. The task book can be found here.
Phase C: The DP Simulator Course will be stamped and signed in the logbook by the training centre. This must be completed before starting on Phase D.
Phase D: The 60 days on board the vessel can be added to the Phase D in the logbook. The 15 DP operations required for Phase D must be completed on the additional DP Operations form. The additional form can be found here.
Phase E: The suitability page of the logbook needs to be completed only once all other phases have been done. This is the last part of the training to get signed off and must be signed by the Master and countersigned by the DPO on board (if the Master does not hold a DP Certificate)
Once everything has been completed and is dated in the last four years, you will be able to complete the online application on the Alexis Platform.
Who can sign off the DP operations for the Self-Elevating Platform(Jack up) Scheme?
The 30 DP operations that are required as part of the Self-Elevating Platform DP Scheme can be signed off by the Master of the vessel.
If you are the Master of the vessel, you will need to have a certificated DPO sign these DP operations off.
I have already started the Offshore scheme, can I transfer to the Self-elevating platform scheme?
Yes, if you have completed the DP Induction Course and hold the grey logbook you can start the new self-elevating platform scheme at Phase B. You will need to download the separate taskbook and the DP operations forms to complete these through your training. You will have four years from the date of your DP Induction Course to complete the scheme and apply for your DP Certificate
